Survey results in 1940 and 1975 indicate that 70% of accidents were attributed to human errors. According to another survey, almost 60% of accidents were due to aircrew members, followed by design faults and maintenance deficiencies being the reasons.

        According to Sears 1959-1983 report, 93 accidents were studied out of which 12% were caused by maintenance errors. Boeing 1982-1991 study reveals that among 232 accidents, 20% were due to maintenance errors. NTSB investigated among 14 accidents that occurred during 2000 were due to the same type of errors.
